8-1/2″ x 14″ ~ Single Sheet ~ 50 per pad This Affidavit should be completed and signed by a non-resident buyer of a vehicle transporting it outside this state under a trip permit, non-resident license plate, or 45-day non-resident military permit. This form is used in order to establish that the sale is exempt from sales tax.
